New Student Application (diocese of brooklyn)

We encourage families to apply online as it simplifies submission and provides parents the ability to track the status of their child's application. 

To begin the Online Application process, Create an Account. Upon account creation, you will receive an email containing a verification link. Once your email address has been verified, log in to your account and Create A New Student Application for your child. You will then have the flexibility to log in and out of your account and access your open application.

After submitting the application, you will be able to track your admissions status at the school by logging in to your account. In addition, you will be able to print the completed application, monitor when the school receives any applicable supplemental forms and, if necessary, reprint the forms.

To complete the enrollment of your child(ren) in Nursery, Pre-Kindergarten, Kindergarten and up, please bring the following:

• Birth Certificate
• Baptismal Certificate (if applicable)
• Complete health records with immunization dates (doctor’s yellow card) 
  • See New York State Immunization Requirements for School Entrance/Attendance
• Records of previous schools (if applicable)
  • Last two (2) report cards (required for students enrolling in grades 1 and up)
• Any other pertinent information that may relate to the child’s performance (if applicable)
  • IEP & 504 copies
• Completed St. Andrew Avellino Catholic Academy registration form

Please complete the forms:

• Health and examination form by child’s physician (return by June 1st)
  • See NYC Department of Education Medical Requirements for Child Care and New School Entrants
• Yellow bus or city transportation form, if transportation is needed (return by June 4th) 

Notes:

• Eligibility for Early Childhood program:
  • Nursery students must be 3 years old by December 31st
  • Pre-Kindergarten students must be 4 years old by December 31st
  • Kindergarten students must be 5 years old by December 31st
• Students in Grades 3 and up, where applicable 
  • Please bring a copy of their Communion, Penance and Confirmation certificates.  
  • All documents must be original.  We will make necessary copies.
